One (1) must be: Deed, Tax Statement, Shared Residency Affidavit or Rental/Lease Agreement

 

Two (2) and Three (3) must include a combination of: Utility Bill, Phone Bill, Cable Bill, Bank Statement Insurance Statement, or Current Government Issued Documents with Same Address (Copies of proofs must be provided)
